FORM 1 -  "Official Certification of Authorization"   (click here to view)

Form 5 and Form 6 can only be viewed and printed using the "Adobe Acrobat Reader".  If you already have this reader installed you can just click on either of the links immmediately above - otherwise please see the detailed instructions below to either:  (1) find out if you have the reader installed; or  (2) install it.  Many computers have this reader already installed.
DETAILED INSTRUCTIONS :

FORM 5:   "Uniform Residential Loan Application" FormAlso known as Fannie Mae and Freddie Mac form number 1003.

You will only be able to see forms 5 & 6 if you have the "Adobe Acrobat Reader" installed.

If you do not yet have the "Adobe Acrobat Reader" installed:

  1. Download the Adobe Acrobat Reader by clicking here.  This free program is commonly used by many web sites and computer programs. If your modem speed is 28.8 it may take about a half-hour to download this program.

  2. Install the program following the manufacturers instructions. Typically this is very simple and only involves saving the file you download to a directory (any directory) on your hard disk and then double-clicking on it from Windows Explorer. This will start the install routine - and you'll just need to click "next" a few times as it walks you through the install.

If you already have the "Adobe Acrobat Reader" installed:

  1. Download the "Uniform Residential Loan Application" by clicking here.
    (The document will either appear in your browser or you'll get a pop-up asking where you want to save it - any directory is fine.)

  2. If the document didn't appear in your web browser (Netscape or Internet Explorer) then you'll need to start the Adobe Acrobat Reader and open the document "form1003.pdf" from where you saved it when you downloaded it.

  3. If the document automatically appeared in your web browser then you'll need to save it by clicking on "File" and "Save As File..." to save the document before you can start the Adobe Acrobat Reader and print - you can save it in any directory.

  4. Start the Adobe Acrobat Reader and click on "File" and "Print", check "Shrink to Fit".  It will print on normal letter-size paper but looks best if you use legal-size paper.

If you're not sure if you already have the "Adobe Acrobat Reader" installed:

  1. Download the "Uniform Residential Loan Application" by clicking here.
    If the document appears in your browser - then you do have the Adobe Acrobat Reader installed and you can proceed as in step 3 above. If you get a pop-up asking where you want to save it - then save it in any directory.

  2. Using Windows Explorer locate and double-click the document "form1003.pdf" from where you saved it when you downloaded it. If the Adobe Acrobat Reader starts up then print as in step 4 above, but if you get a window asking which application you want to use to view the document - then click "cancel" and proceed to the instructions above on downloading and installing the Adobe Acrobat Reader.
